The End of Life Coordination Centre

The End of Life Care Coordination Centre is a single point of access for all health care professionals who are caring for patients towards the end of their lives. They provide an overview of the health and social care services that are available in North Somerset. This enables them to tailor a package of care to meet patient’s needs, allowing them to be cared for and die in their place of choice.
