Bristol Care Coordination Centre for End of Life Care

The End of Life Care Coordination Centre coordinates care for patients in the last three months of life across Bristol. It helps people to access the following services:

Palliative Care Home Support Service
Personal care and emotional support for patients who wish to die at home and are in the last six weeks of life. The team is based at Avonmouth but care is carried out in patient homes in Bristol and South Gloucestershire.

Fast Track (FT) Service
Assessment for funding packages of care, nursing home placements/end of life beds in Bristol. This service is for patients who are rapidly deteriorating on a daily basis with a prognosis of 12 weeks or less and need funding for packages of care.

Marie Curie
We allocate Marie Curie nurses to provide night care on a daily basis, according to clinical need, for patients in the last three months of life.

Hospice at Home (St Peter’s Hospice)
We liaise twice daily with the Hospice at Home Team to ensure continuity of care for patients considered to be in the last two weeks of life.
