Primary Care Case Finding Pilot

Here at SWAG, we recognise that our local Primary Care Networks (PCNs) and GP practices are best-placed to run local pilots that support proactive earlier diagnosis, yet, often lack the additional funds to set up and run projects.

That's why we have launched this new Primary Care Case Finding Pilot, looking to fund projects across primary care that could result in earlier diagnosis of cancer.

Cancer case finding involves actively searching for suspected cases within known high-risk groups or during a clinical encounter for, and referring individuals on for further testing and follow-up of the cancer pathway, to catch different types of cancer early, often before they are showing symptoms, or when treatment is more effective.  This could involve searching patient records for specific health conditions or warning signs, in order to flag them for a targeted and proactive pathway.

Many people have contact with their primary care GPs for routine or other healthcare needs, yet often, signs of cancer can go unnoticed or undetected. We are calling on GPs or PCNs to submit applications for projects that seek to proactively identify early cancers in their patients.

We are holding two drop-in online sessions in October and November for anyone who wants to find out more or has any questions on the application process.

Who can apply?

Any PCN or GP practice within Somerset, Wiltshire, Avon, or Gloucestershire is invited to complete the application process. Please note, that if successful, you will be working alongside the SWAG Early Diagnosis Project Manager to deliver your pilot.

What is the application process?

There is a total of £50,000 funding available - each PCN or GP practice can bid for a portion of this funding to conduct their pilot.

You must complete the Expression of Interest application, either via the word document below or online by 28 November 2025. 

SWAG Cancer Alliance will shortlist applicants and select a maximum of four projects to pilot. Projects are expected to run for 12 months to enable effective data collection and evaluation.

Timeline

  • 1 October: Applications open
  • 28 November: Applications close
  • December 2025: Panel reviews applications
  • 19 December: Applicants will be notified of the outcomes
  • January 2026: Mobilisation meetings with SWAG Project Manager
  • February 2026: Projects
